Introduction: The incidence of unintentional weight loss is about 0.6% to 7.3% per year. There are different definitions without a precise consensus. In Mexico there are few studies on the incidence and characteristics of patients with unintentional weight loss. Objectives: To determine the main clinical manifestations and etiologies of patients with significant unintentional weight loss from 2017 to 2019 in our hospital, time of diagnosis, costs per hospitalization, as well as clinical and etiological differences in patients with unintentional weight loss classified by two definitions used. Methods: Observational, descriptive, cross-sectional, single-center study. Through retrospective chart review of all patients admitted to hospitalization with the diagnosis of unintentional weight loss from January 2017 to January 2019, patients were classified into two groups: current definition or group 1 with 5% body weight loss in 6 months and group 2 or initial definition with 5% body weight loss in 3 months. All patients with weight loss greater than 5% in 3 months were included. Descriptive and inferential statistical analysis was performed. Results: The most common etiology found was infectious with 12 patients (24%), followed by neoplasms with 11 patients (22%); 8 patients (16%) had no diagnostic conclusion. Of the total of 50 patients, 9 (18%) died after definitive diagnosis during hospitalization. The most common aggregate symptom was fever in 52%. Ferritin and 2-microglobulin were found in addition to other previously studied markers. 13 patients met the definition established by Bilbao, of which 10 patients had an organic cause. Conclusions: In our study we found a high prevalence of organicity, with the main cause being infectious etiology followed by neoplastic. The use of a strict definition of unintentional weight loss could exclude patients who require a structured diagnostic approach.
